Symptom:
End User Acknowledgement (EUA) is displayed to end users even after it is disabled in Identities and/or globally under End User Notification section

Conditions:
The problem happens when surrogate type of "Session cookie" was configured for EUA under End User Notification section before disabling and when websites are configured to 'Warn' in access policies
