Description (partial)

Symptom:
Similar to CSCum66724, we see an environment with a single B480M5 with 1440 and 1480 adapters running on 4.0(4g) seeing duplicate VIFs. These duplicated VIFs will show as down, and raise alerts on the customers environment.

Conditions:
Single Domain running 6454, only one B480M5 with 1440 and 1480 VICs, running HANA application. 
UCSM and Blade both on 4.0(4g).
