Description (partial)

Symptom:
Installing a M5 server for the first time, service profile may fail association with "Connection Placement Error". Upon further investigation from UCSM, you will not see a path created for both A and B fabric.

Run this command from UCSM CLI to confirm symptoms:

UCS-FI-A# show server status detail

Working:
Server 2/4:
    Slot Status: Equipped
    Equipped Conn Path: A,B
    Equipped Conn Status: A,B
 
Problem:
Server 2/5:
    Slot Status: Equipped
    Equipped Conn Path: A <<<<<
    Equipped Conn Status: A

Conditions:
6400 Fabric Interconnect and 2408 IOM

This occurs after initial discovery of a blade. You can re-ack the IOM on the missing path to fix the issue, however, new blade installations may see same issue.
