Description (partial)

Symptom:
The issue is seeing when we use an input type of "File upload" in a workflow. 
When the file is already uploaded, if there is a change in the file and the file have the same filename will not overwrite/update the file that was uploaded for the first time, the file that was added at the first time will remain in UCSD, and will not update/overwrite that file if exists already in apache multipart upload path in UCSD.

Conditions:
UCSD 6.7.4.x or 6.7.3.x
Input type of "File Upload".
The file uploaded does have the same name as another previous file uploaded.
