Description (partial)

Symptom:
During upgrade, when the product.img is being mounted and extracted, the following behavior has been observed intermittently:
When using Upgrade option from GUI:
The upgrade will fail, and the system will become unusable after failure. In the install log, the last line will look like this (version may be different):
refresh_upgrade|unarchiving /common/download/12.5.1.21900-35/product.img to /common/mnt/product_img|<LVL::Debug>

If upgrading using CLI, the upgrade will fail in the same place, but more details will be in the install log, like this:

Upgrading to any 12.5.1 CER version
