Description (partial)

Symptom:
After a volume based service software update failure, downstream recovery can fail when performed after an ESC failover.

Conditions:
The following workflow can result in latching this condition:

1> Trigger a volume based service software update.
2> An underlying VIM failure causes the software update to fail (one or more VM_ERROR_STATE are raised). 
3> Recovery is triggered to recover the service.
4> Recovery completes and service VMs return to service (VM_ALIVE_STATE).
5> Send service update with ESC failing over before update completes.
6> Service update fails as expected.
8> Attempt to resend or revert (recovery) the service update (fails).
9> At this point service volumes can’t be managed so VM state can no longer be progressed.

When this condition latches the following log will surface confirming underlying VM database access violation:

ERROR: duplicate key value violates unique constraint "pk_vmstoredstatestable"
