Symptom:
Notice APD events and Cluster may go down.

Conditions:
The exact condition under which this problem is seen is still unknown.  Some of the possible causes are mentioned here.
1. Excessive packet loss in data network. 
2. Controller VM server threads stop responding.

The above condition stalls mirroring operation and cluster slowly becomes unresponsive to user I/O. In some cases all copies of data may be lost temporarily causing cluster to go down.
