Description (partial)

Symptom:
Extensions and ACG fields are grayed out at Quick user/Phone add page in Subscriber

Conditions:
This issue is reproducible only on subscriber.

Go to ccmadmin - User management - User /Phone add - Quick user/phone - add new - enter last name, user id, select feature group template ,user rank,Click + button in the "user is a member of" and select one acg, Click + button in the extension and select an extension.
Now click save. It will be saved successful but ACG and extensions fields will be grayed out.
Again click + and add acg and extension - click on save. When we click save button again and again, ACG and extension fields are displaying and hiding the newly added contents.
