"" error at Add new phone page

Goto ccmacdmin - Advanced feature - MRA service domain - Create service domain.

Goto BAT - Phone - Phone template - Create a phone template which supports On-Premise Onboarding Method (eg: 8861) with created MRA service domain. (Enable -Require Activation Code for Onboarding and Allow Activation Code via MRA)

Go to Device - Phone - Add New - Select BAT Phone Template - Select the phone template and click on NEXT - Now click on Generate New Activation code.

It navigates to add new phone page and throws ""

Issue is seen on 12.5SU3 and 14
