Description (partial)

Upgrade should wait until a monitored VM reaches ALIVE or ERROR state. If upgrade is done in an intermediate stage of VM , where system is trying to recover and test for reachability, it will cause some of the other VMs to remain in shut state after upgrade.

Symptom:
In some cases, partial set of VMs can remain in shut state after a successful upgrade.

Conditions:
Upgrade done when a "monitored VM" is still in "INERT" state. 
INERT state is when a VM has been booted up but the reachability check is in progress, until it is marked as ALIVE or ERROR.
