Description (partial)

In previous version 11.X, each node has its own ITLRecovery certificate. In version 12.X the ITLRecovery is the same in all the nodes (ITLRecovery of the Pub). In order to meet the new condition for version 12.X, the ITLrecovery certificate is regenerated automatically during upgrade to CUCM 12.X. 

The CTL file will have the old ITLrecovery certificate and will still be signed by the CCM certificate (the CTL file is not updated automatically after the upgrade).

Since the ITL file was updated at the moment of rebooting the servers and now is signed by the new ITL recovery, which is not included in the old CTL file, the phone will not install the new ITL file.

Upgrade of a mixed-mode cluster to 12.X.

ITLRecovery certificate date matches with the date of the upgrade. 
ITL last modified date in the show itl output is a date after the upgrade.
CTL last modified date in the show ctl is a date before the upgrade.
CTL is still trusted and installed by the devices.
ITL is not installed error is present in the devices.
