Description (partial)

Symptom:
M.2 boot disk goes missing during operation and may result in host OS becoming unresponsive. Once the host if finally rebooted (as part of an attempt to recover the server), the M.2 disk disappears from the inventory and host OS is unable to boot. The issue persists are decommission and re-acknowledgment of the server (in this case, C-Series server integrated into UCSM).

Conditions:
This issue was first observed during normal server operation (no recent changes or known triggers). HXAF240-M5SX (C240 M5S) running UCS release 4.0(1c) and M.2 disk firmware D0MU054.
