Description (partial)

Symptom:
After an upgrade some ports may not come up.

You can validate the defect by checking the port logs and see if there are any failed read errors using following command:

vsh_lc -c "show platform internal tah event-history trace" | grep "mdio rd failed" 

Example Broken state:
247) Event:E_DEBUG, length:97, at 2020-07-31T13:38:38.580847000-05:00
[808595506] tahusd_mifpga_mdio_reg_read/247: mdio rd failed inst: 8, port: 0, dev: 0x1e, offset: 0x400d

Conditions:
This issue can be seen after upgrading or after some port flaps.
