Cisco Bug: CSCvt30719 - Issue with Phone Migration page layout

Symptom: Positioning of MAC Address field in the page gives an impression its required only for migration using Phone Type. MAC address is a mandatory field in all modes of migration from the UI. Conditions: 1)Login to CCMAdmin. 2)Access the path Device->Phone. In Find and List Phones page, select 9971 phone already present in the node. 3)From related drop down box, select Phone Migration. 4)In Phone Migration page, enable radio button against Phone Template text and click on Save button. Warning message indicating MAC Address is required is seen Ideally either Phone Template option or Phone Type option can be selected. As Phone Template option is selected.Phone Type option is in disabled state. However, page still prompts to give MAC Address part of Phone Type option. MAC Address is a mandatory field. Concern is the positioning of MAC Address field in the page. Phone Template or Phone Type Radio button as of now is seperated by "or" text. There must be space/gap after the Phone Template/Type section indicating the user that post selection of either Template/TYpe post MAC Address field must be populated.
