Symptom:
PCIe width degradation is seen in the following Qlogic adapters after upgrading to firmware version 08.04.21.02.02 in release 4.1(1c):

Cisco® FastLinQ QL41162H 10GbE adapter 
Cisco FastLinQ QL41132HORJ Dual-port 10G RJ45 2x10GBase T OCP 2.0 
Qlogic QLE2692 dual-port 16G FC HBA

Conditions:
Issue is seen in Cisco UCS C125 servers equipped with the following Qlogic adapters:
Cisco® FastLinQ QL41162H 10GbE adapter
