Symptom:
Software upgrade from tftp failed due to timeout.

Switch#request platform software package install switch all file tftp://xx.xx.xx.xx/cat3k_caa-universalk9.16.xx.xx.SPA.bin
--- Starting install local lock acquisition on switch 2 --- Finished install local lock acquisition on switch 2

Downloading  file tftp://xx.xx.xx.xx/cat3k_caa-universalk9.16.xx.xx.SPA.bin to active switch
FAILED: Failed to download file tftp://xx.xx.xx.xx/cat3k_caa-universalk9.16.xx.xx.SPA.bin: Timed out

Conditions:
1.run "request platform software package clean switch all file flash:"
2.upgrade use "request platform software package install switch all file tftp://xx.xx.xx.xx/cat3k_caa-universalk9.16.xx.xx.SPA.bin"
3.if step 2 success ,then reload the device .
4.run "request platform software package clean switch all file flash:" again.
5.downgrade use "request platform software package install switch all file tftp://xx.xx.xx.xx/cat3k_caa-universalk9.16.xx.xx.SPA.bin"
6.if step 5 success, then reload the device .
7.repeat above steps 2 times or 3 times ,we will see the issue .
