Description (partial)

Symptom:
On the System > integration > users > users and roles section of the FMC, the access admin role is unchecked. Also, each time you try to log into the FMC GUI it tells you that you have insufficient credentials as if you are not putting in your password correctly.

Conditions:
All FMC versions affected.
External authentication DOES NOT have to be enabled
User DOES NOT have to be the admin account but any account with admin privileges
