Description (partial)

Symptom:
Self-Provisioning IVR is not working if End user has more CTI controlled  and EM controlled profiles

Conditions:
Goto ccmadmin - Device - Device settings - Device profiles- Create three device profiles. 

Go to user management - User settings - user Profile - set Limit Provisioning once End User has this many phones is 3. 

Create an End user with created user profile and assign 2 CTI controlled device profiles (Under device information) & one controlled profile (Under Extension mobility). 

Now perform self provisioning IVR , it says maximum number of devices reached.

It works fine if end user doesn't have device profiles.
