Description (partial)

Active MIO becomes unable to access a device (CAF) in a DPC card.  
The PCIe switch is locked up for that path.

PCIE switches on MIO/MIO2 management cards have trouble handling surprise down events.
Surprise down events may happen in a number of situations such as:

- Module removal
- Card failures
- Card reboots
- MIO switchovers
- DPC migration 
- Card upgrades
