Description (partial)

Symptom:
The Cat 9500 & 9600 devices may crash during the insertion and removal of the QSFP transceivers. Once the crash happens, all interfaces may shutdown on the standby device and may require a reboot to recover.

A sign of the bug is that the IOMd process crashes.
