Description (partial)

Symptom:
Line Card Failure Signature  - This failure event is accompanied by the following characteristics:
 1. Ethernet Out-of-Band Connection (EOBC) heartbeat timeout, causing module to be automatically restarted to avoid traffic impact 
2. Three failed attempts to boot up the module
3. After third attempt to boot up module, line card is powered down and declared failed.

Conditions:
•	This issue has been root-caused to the failure of a power stage component, where the feedback loop to the power sequencer stage can sometimes be too slow for the input to throttle down in time
•	This failure event is not dependent upon operational hours, and it can happen at any time during operation
•	The impact of this failure is an immediate shutdown of the line card.  With the exception of packets-on-wire during failure, no traffic impact is expected
