Description (partial)

Symptom:
A C9500 High Performance switch will suddenly stop detecting new SFPs added.

Logs are seen upon insertion-removal but SFP is not see on "show inventory" nor interface goes to err-disabled:

*Nov 15 20:11:42.137: %TRANSCEIVER-6-REMOVED: R0/0: iomd: Transceiver module removed from TwentyFiveGigabitEthernet1/0/2
*Nov 15 20:11:51.151: %TRANSCEIVER-6-INSERTED: R0/0: iomd: transceiver module inserted in TwentyFiveGigabitEthernet1/0/2

Any SFP already working on the C9500H will start failing after removal-insertion of the SFP.

Conditions:
Working SFPs will not display idprom information on an affected switch. These SFPs will not be detected if removed-reinserted:

On an SFP not being recognized, following errors will appear on IOMD traces after inserting the SFP:

[iomd] [7766]: UUID: 0, ra: 0, TID: 0 (ERR): Verify Checksum failed: sum:e1 checksum:ff
