Description (partial)

Symptom:
Ports on standby switch 9500-24Q stackwise virtual pair running 16.12.1 and using QSFP-40G-SR4-S qsfp may remain down/down (notconnect) after reload even though the breakout commands have been correctly configured

In some cases removing and reapplying "hw-module breakout module 1 port <> switch <>" breakout commands might bring the links back up

Conditions:
Reload of 9500-24Q stackwise virtual pair running 16.12.1 may cause QSFP ports on standby switch to remain down/down
