Description (partial)

Symptom:
After adding a local audio add-in or additional video endpoint locally from a video endpoint (IX 5000 specifically), users may see black screens and failed presentation - overall unexpected video behavior - after merging the additional audio or video line.

Conditions:
Commonly seen on IX5000 and other Immersive endpoints, 8.2.3 and 8.3.0, using CMS as bridge.  

+ Video endpoint (IX 5000) configured in CUCM to "Use MRGL" under "Multipoint Mode" in Vendor Specific Configuration.
+ IX 5000 dialed into an existing CMS bridge call tries to add a local audio add-in or add an additional video participant from the IX 5000.
+ An adhoc conference is created on CMS, causing a cascade between original conference and adhoc conference, resulting in black screens/failed presentations, and overall unexpected behavior.
