Description (partial)

Symptom:
A module fails ISSU/D due to hardware errors but is still allowed to come online after being reset. The following syslog message is logged after the ISSU/D failure:

  %VMM-2-VMM_SERVICE_ERR: VDC1: Service SAP ACL manager for slot 2 returned error 0x804e0001 (Operation not permitted) in upgrade sequence

  %MODULE-2-LCM_UPGRADE_OVER_FAIL: Upgrade over message returned 1 0x804e0001 for SAP Im SAP
  %USER-2-SYSTEM_MSG:  "Module(s) 3, 4, 7 still need to be upgraded" - new_installer

Conditions:
This issue only occurs when a module with latent hardware issues ungoes ISSU/D.
