Description (partial)

Symptom:
The mobile and desktop previews do not display Phone number and SMS provider fields on the self-registration form when "Send credential notification automatically using: SMS" option is chosen in the Settings tab.
However, both fields are present on the registration form when using Portal test URL.
Tested on ISE 2.3 no patch, ISE 2.2. patch 5

Conditions:
ISE 2.x when trying to customize a self-registration form
