Cisco Bug: CSCvh72242 - Getting Blank white space when trying to Add or Update Users in "Quick User/Phone Add"

Symptom: Trying to either Add New or Update User and Phone relation from the "Quick User/Phone Add" page leads to getting a Blank white space where the fields and boxed would actually be. Conditions: Have the Username typed upon logging in with different letter case (lower or upper) from what's configured under the End / Application User page. For example, Username initially configured is "John.Doe" but upon logging in, "john.doe" was used. Same with "admin" versus "ADmin".
