Description (partial)

Symptom:
Caller may hear dead air or the call may get dropped depending on the call flow.

Conditions:
In a call-flow involving CVP whch transfers the call to voicemail, it was observed that the mid-call re-invite and the notify message went out of order(reversed CSEQ number) from CVP to Callmanager.  Instead of responding to the CVP with a 500 error message, Callmanager sent the error response to the CUSP which was used originally to route the call to CVP.
