Description (partial)

Symptom:
The issue can show up as :
1. Connection to storage target is lost when FC link is removed from the SAN Port-channel
2. LUNs visible through fNIC may go offline with the message "Medium access timeout failure. Offlining disk!".

Conditions:
Seen on UCS Mini running FC End-host mode, however may affect other environments.
Any condition that could result in several IO timeouts and aborts could result in disk offline.
