Symptom:
Cisco ACI APIC may not properly upgrade.

loop devices can be seen on teh apic via a "df -h":

In some cases, attempting to manually detach the loop devices via root with command " losetup -d /dev/loop#" will fail.

Conditions:
A Cisco APIC during upgrade operation may fail.  When this occurs, a fault (fltMaintUpgJobUpgradeFailed - F0105) will be generated.

This may occur if there was a previous upgrade attempt and error (for example if the apic iso was corrupted).
