Cisco Bug: CSCut16603 - Gen4/5 port not able to be brought up (error disabled) on an IVR switch

Description (partial)

Symptom:
A port cannot be brought up. If it is a member of a port channel the whole port channel is brought down. The following type of syslog errors are logged:

  %PORT-2-IF_DOWN_ERROR_DISABLED: %$VSAN 1%$ Interface fc1/1 is down (Error disabled)

The show port internal event-history errors command show the following type of messages at the same time as the syslog message above:

    [102] mts_acl_update_e_port_entries: if_index fc1/1 (0x1091000) acl programing failed status (0x12)
    [102] pi_fsm_port_attr_change_init: Ifindex (fc1/1)0x1091000, Err disabled event (PI_FSM_EV_ACL_EPORT_PROG_FAILURE)0x21

Conditions:
This issue only occurs when all of the following conditions exist:

- the affected port is on a generation 4 or 5 module
- IVR is configured on that switch
- there is no route to a destination VSAN
