Description (partial)

Symptom:
Front Panel GigE goes offline and FPGE interfaces would disappear from the configuration.

This can lead to router crash as well.

Conditions:
The issue is specific to the following SFPs and to ISR4451/ISR4431 routers.
GLC-SX-MMD, GLC-LH-SMD, GLC-EX-SMD, GLC-ZX-SMD, GLC-TE

The issue hits very rare when user tries to do SFP OIR or tries to install a new SFP. The communication between router and SFP hoses that causes the FPGE module goes to offline.
