Description (partial)

Symptom:
Cisco Nexus 9500 may have issues with the 40G spine line card or the Fabric Module where they may get into a non-functioning state.  When this occurs, the output of "show module" on the switch which should be in an "ok" state may display "powered-dn", "unrecoverableerr", "failed" or have not entry.

Conditions:
Cisco Nexus 9500 running in ACI mode or standalone mode goes through diagnostic tests when the chassis is powered on, a module is inserted into the chassis as well as periodically on an operating system.  Under rare conditions if a failure in this self diagnosis occurs, the module will not come into a functioning state.

In the case of the 40G line card and the Fabric Module, it would either be the LC or the FM which may have a software or hardware issue.
