Description (partial)

Symptom:
The initial symptom of this bug is that one VIC adapter of a dual VIC server will remain in an Activating state, from UCSM, after a server firmware upgrade.

As UCSM still believes the adapter is in an Activating state, the associated server is exposed to an "unexpected" reboot in an attempt to complete the upgrade. This reboot could be triggered after making a Service Profile altering configuration change, or after UCSM events such as HA fail-over, Infrastructure firmware upgrade, process restart, etc.

If this unexpected reboot occurs, the adapter will still remain in an Activating state and the workaround must be performed to correct this.

Conditions:
1) B-series server w/ dual VIC adapters
OR
UCSM Integrated C-series server w/ dual VIC adapters

2) One of the two VIC adapters in the server will remain in an "Activating" state after a firmware upgrade

3) The two adapters are being managed by different FI managing instances
