Description (partial)

Symptom:
AXL request is being lost somewhere between Tomcat, DB, and AXL on the CUCM side, 
resulting in updates being deleted due to missed port responses, 
which, in turn, removes all ports created due to the missed AXL responses

/var/log/active/tomcat/logs/dbl/log4j/ 
has consecutive log entires in same ms.
