Description (partial)

Symptom:
Provisioning will stop and will be stuck in pending Active state with the following duplicate item error in the logs.

Message: ap_dim01_prov_update_member_pkey_map: SQL Error = [50504] Attempt to insert duplicate item to equipment mapping. [Source: [tr_dim_agent_skillgroup_member_inserted_updated_validate]] Source: .Net SqlClient Data Provider

Conditions:
Scenario occurs when two users submit the same request one from one webserver and one from the other at the same time. Due to small lag in SQL server replication by doing it on both sides and the record therefore not being in the DB when it is submitted on each side the UI can not detect this as a problem and inserts it. When these record are then replicated and provisioning runs the logic detects the issue and causes the error and provisioning stalls
