Description (partial)

Symptom:
Both Fabric Interconnect, Primary and Secondary, reboot with the following reset reason:
UCS-FI(nxos)# sh system reset
----- reset reason for Supervisor-module 1 (from Supervisor in slot 1) ---


1) At 860047 usecs after Wed Sep 24 10:31:16 2014
    Reason: Reset triggered due to HA policy of Reset
    Service: npv hap reset

NPV FC2 memory counter increments as seen with the output of:

NPV Error messages show duplicate FLOGI attempts:

4) Event:E_DEBUG, length:258, at 327206 usecs after Sat Feb 21 09:07:34 2015
    [118] S(1,vfc1453) Received FLOGI/FDISC from pwwn 20:00:00:25:b5:a1:a0:37 from interface vfc1453 when there is an entry on interface vfc1453 fo the same wwpn, with fc_id 0x01aeae. Rejecting the frame with R_BUSY and implicitly logging out the previous entry.

Conditions:
Cisco UCS Manager running 2.2(1d) with UCS-FI-6296 managing14 chassis during normal production run-time.
