Description (partial)

Symptom:
Rack servers fails discovery with message "scsi controller must be in slots 3 and 4"

Conditions:
Observed in following hardware setup 

UCSM 2.2.1c
CIMC 2.0.1b package
Rack servers are directly attached to FI

Rack server ( example C 240 M3 S2 ) is installed with two RAID controllers ( Mezz and LSI PCI card )
