Cisco Bug: CSCuq69550 - Cisco 8945 IP phones cannot upgrade or downgrade firmware

Symptom: • Reset Factory was done, however if we login into the phone via SSH and run the command show phone_info, we see that the phone has an active and inactive load as below: [CP-8945] show phone_info Model number : CP-8945 Chip ID : 111970b0(NEW) IP address : 10.80.100.60 MAC address : 1C:E6:C7:9A:86:BD BT MAC address : 1C:E6:C7:9A:86:BE Boot version : 0.0.2.0 Active application version : SCCP 9-3-4-17 Inactive application version : SCCP 9-3-2-11 Last upgrade time : Feb 04, 2014 09:51:25 • Erasing the security settings was done. • Restarted the TFTP servers. • The customer had took one of the affected phones to a lab with the same CUCM version and latest device pack installed on it and faced the same problem. • Tried to downgrade the phone to version 894x-sccp.9-2-3-5.cop.sgn and release 9.3(1), however same problem • Tried to change the Firmware to SIP, however same issue. • Collected a Sniffer capture from the phone. and saw that the phone successfully pulls "loads" and "bin1" files however it does not request subsequent "bin" files, checked the ".loads" file, and it had all the needed bin file names, so it seems that the phone stops requesting the "bin" files at the time of the failure. • From phone logs, it looks there is a problem with space on the flash or write permissions on the flash or the flash is corrupt, see below: MTD Erase failure: offset:0x0 dev_mtd= /dev/mtd8, Input/output error MTD Erase failure: offset:0x220000 dev_mtd= /dev/mtd8, Input/output error bad block at offset:0x0 rc=1, fd=72 Skipping bad block at offset 0x0 g_partitionSizeLeft_3M=0x240000 bufsize=0x20000 isKernel 1, fp = 0x1bdcc10, ftell = 0 00:00:01:23.876 - I/kernel: nand_erase: attempt to erase a bad block at page 0x00000ac0 Software Upgrade Failed - Exhausted partition /dev/mtd8 looking for good blocks 20:50:45, 08/01/2013 : Upgrade Failed Conditions: • Upgrade phones Firmware. • CUCM upgrade from version 8.5 to 9.1.
