Description (partial)

Symptom:
A TelePresence MCU unexpectedly rebooted. The backtrace shows a failure in the debug_vstream_disconnect function.

Conditions:
This reboot is triggered in a very specific scenario when a conference is destroyed whilst and endpoint is presenting H.263+ content.

Order or events:
First endpoint connects supporting only H.263+ content
One endpoint joins that is not capable of either H.263+ or H.264 content
At least 2 more endpoints join, supporting both H.264 and H.263+ content.
One of the last two endpoints starts presenting, and is still presenting when the conference is destroyed.
