Description (partial)

Symptom:
B200M3 running firmware 2.2.2c, local SD esxi 5.5 boot and local HDDs being used as JBODs
when a VM running on one hdd is cloned to other hdd, we get error below

Conditions:
issue is with LSI firmware & esxi 5.5
issue doesnot happen with esxi 5.0
