Description (partial)

Symptom:
When picking up the affected phone, dial tone can be heard. But after inputting dialed number, the phone does not have any reaction, it will not dial out.
    When another phone calls this phone, the affected phone will not ring. The calling party can hear only dead air.
    If phone is reset, the issue will be gone.

Conditions:
3905, firmware 9-4-1, CUCM 9.1.2
