Symptom: Unable to login via TACACS+ and assertion failure seen when access is gained via logging in with local fall back password. Rare situation where MIO would need to be under a lot of stress. Conditions: MIO has to be overstressed in order to hit this problem. It can be reproduced by removing Gi routes and sending heavy traffic to the box with demux on MIO. A lot of traffic will be redirected to the MIO in this case when the there are no available Gi routes to the DPCs, causing a lot of stress on the MIO. MIO switchover also seems to trigger this issue while the MIO is under heavy load.
