Symptom:
phone can't recognize usb device after lots of plug in/out

Conditions:
phone load: sip88xx.10-2-2-4
plug in/out usb device(hub, headset, hub with headset) for lots of times, phone can't recognize the usb device.
or the usb device has been pluged out, but accessory menu still have the usb device info
