Description (partial)

Symptom:
When selecting a phone from Product dropdown, the fields are not reflected correctly. For example, when selecting CTI Remote Device, CUCDM update the form to reflect the fields that are related to the CTI Remote Device, however, some field such as 'Owner User ID' is still missing and several inapplicable fields are still shown up.

Conditions:
When adding phones to an existing subscriber under Subscriber Management -> Subscriber -> Phones
