Description (partial)

Symptom:
Audio path icon on the volume status bar is displaying wrong icon (BT headset) even though the actual audio is to built in speaker.

Conditions:
BT headset paired/connected.
Phone call made but Audio is not able to be set on BT headset.
(typically, this is due to BT headset out of range so not able to allocate radio link).
U/I prompts user to either End the call or Switch to Speaker.
If user selected either, the icon on the status bar remains as BT headset.

The next call will be to speaker (as at this point, BT headset should be considered as disconnected)
So the icon should change to Speaker.
