Description (partial)

If you have an identity group and go to Administration->Web portal management->settings-Guest->Guest Roles configuration and add this group to "activated Guest" list, then all guests created with that group will be immediately "active" and not "awaiting initial login". This is the normal behavior of the feature.
However, if you move the group back to the normal Guest list and save, newly created guests will still be immediately active and not "awaiting initial login". If you reboot ISE, then the behavior correctly changes back to a normal guest behavior.
