We have reviewed our test cases and we test to ensure that vmcores are being generated and do not see anything wrong with the settings or anything else with the machine at the customer.

Symptom:
No vmcore is generated at the time of kernel crash.

When a crash occurs, the kdump kernel is launched. Once the megaraid driver is loaded the driver fails to find the system disk drive. The driver will report and SCSI commands are not completing and attempt to reset the disk channel and retry the command(s) a number of times.

e.g.

megasas: reset successful

megaraid_sas: no pending cmds after reset
megasas: reset successful

megaraid_sas: no pending cmds after reset
megasas: reset successful 
....

After a while the megaraid driver will give up, the initrd init script fails, and the machine is rebooted.

Conditions:
CDSTV 2.6.32 kernel, CDE460 hardware, and a kernel crash.
