Description (partial)

Symptom:
User makes call or is on existing call and presses either transfer or conference.  User then inputs desired digits, but nothing happens.  User can then resume original call and end call, but transfer/conference fail with no feedback to user.

Conditions:
CP-3905 on 9.4(1) or later after either long period of no phone reset or heavy usage.
