Description (partial)

Symptom:
Cox Las Vegas restarted SI and Cam processes on the EC. This put DRM into recovery. While DRM was in recovery it sends another createSession to qamManager of an session id that was already turndown (release). DRM logs the following error: "Recovery failed. User Intervention Required. Teardown Session Id/Start-Time 00:21:BE:55:80:4A/39683262/1403692155 and if applicable, restart?. We are seeing the same error for over 400 sessions that were released. The problem is that qamManager gets the second createSession for the same session ID, but does not get a release from DRM, thus causing stranded sessions on the NSG QAM.

Conditions:
Orphaned sessions were left on NSG QAMs, which could potentially result in the occasional VOD black screen.
